Act on Alzheimers
In 2008, the Park Nicollet Foundation and the Jewish Family and Children’s Service of Minneapolis NORC project (nurturing our retired citizens) co-developed
the Successful Aging Initiative (SAI). Mid-2012, members of the SAI formed a St. Louis Park ACT on Alzheimer’s action team of 12-15 individuals, including aging services professionals, a retired physician, attorneys, social workers, senior residents, and business owners. The team envisioned “helping prepare the community for Alzheimer’s disease by enhancing the infrastructure and supports currently available and giving community members access to resources.”
Jonathan said, “We felt all of the community sectors were important to assess. One area we de-emphasized was attorneys and financial advisors, since we had that point of view represented through our team members. Two areas we saw as priorities were professional long-term care service and support providers (home care, assisted living, housing with services, nursing facilities, adult day) and the professionals within the Park Nicollet health system – both physicians and social services staff – who play a significant role in serving seniors in our community.”

As the team implements their work plan, Jonathan notes, “We are very excited about the positive community engagement we've experienced, which will help guide our team's future planning.”
